Most surcharges are valid for all departures (within a market/country) and all destinations; a search can be further narrowed down by inserting the origin- and destination fields.

The Market, Origin and Carrier can be pre-defined in the ‘User Preferences’ which can be adjusted at ‘settings --> user preferences’.
See the help section at that page for more information.
Origin and Destination should be based on the 3 letter-code.
When typing, a list of possible codes are displayed from which a selection may be made.
